CILSF partners with disability rights activists across the city, state and country, plus maintains relationships with elected officials, in order to foster our independence, integration and civil rights.

Transitions: Community & Youth
If you’re looking to transition from an institutional setting, such as a nursing home or a rehabilitation center, into community or home based living, our Independent Living Advocates can help you develop a customized plan and recommend services and local resources that will guide you throughout the entire process.
If you’re about the graduate from high school and need some guidance as you transition into college and/or the workforce, do not hesitate to contact us today! We have several programs and services that can give you a head start on your preparations.
